The news is fast and furious, with media outlets racing to be first to report breaking stories. The problem is, often they get it wrong. They start to cover a story without doing their own first-hand reporting, or they make assumptions.
In the aftermath of the shooting of Arizona Congresswoman Gabrielle Giffords, for example, many news organizations reported that she had died without verifying eyewitness accounts with authorities. They based their information on panicked bystanders who described her as “slumping in the corner with an apparent gunshot wound to the head.”
